Notes:

As the Awards Committee consists of reviewers with diverse areas of expertise, an effective nomination is not just a CV. While a CV may be submitted separately from this form to support a nomination, a complete nomination must include answers to the questions in the form below; these responses will be the primary focus of the review. Incomplete nominations will not be considered. Please do not include hyperlinks  — all information to be considered should be included in the nomination form. Supporting materials, such as a CV or other documentation, may be sent to medalum@virginia.edu or attached to the form for consideration.

The selection committee can opt to not select a winner in a given year if it determines there are no qualifying or deserving candidates.
